Thank you for taking a moment out of your busy day to fill out our Response Partners questionnaire. We are constantly updating our vendor database. We use this database to help our employees find assistance all over the country. The more information you provide us, the more efficient our system will be!

You can access the Subcontractor questionnaire and guidelines with the links below. The forms in Microsoft® Word® 2000 format can be completed and returned in any one of the following ways:

• Print and complete the forms by hand -- Mail or fax to return.
• Complete the Microsoft® Word® forms electronically - E-mail the forms using File/Send To/Mail Recipient (As Attachment). [This will create a new e-mail message through your e-mail account and attach the questionnaire form to the e-mail. Send the e-mail to csalin@hepaco.com. Repeat that process for the company form and each of the office forms.]
• Download the forms and complete them electronically at your convenience -- Print and return forms via mail or fax OR e-mail them using the instructions given above.

If you do not have access to Microsoft® Word® 2000 or better, you can download the forms in Adobe® Acrobat® format (pdf) and complete them by:

• Download the forms and print them out. Fill them in and return forms via mail or fax it to (704) 598-9224.

You will need Adobe® Acrobat® Reader to view and print the Adobe Acrobat files.
